Senior Long Term Care Services
Stearns County Human Services provides ongoing care coordination services for persons age 65 and over. The care coordinator assists the older person to access needed community supports and services such as adult day care, assisted living, homemaking, home health care, etc.
To qualify for county services, persons need to be eligible for either the Alternative Care program or care coordination through their Medical Assistance managed care organizaton (that contracts with Stearns County), including Elderly Waiver care coordination. Eligibility for these programs includes:
Alternative Care
- Age 65 and over
- Without home and community-based services, the person would require nursing facility level of care
- Would be financially eligible for Medical Assistance within 180 days after admission to a nursing facility
Elderly Waiver
- Age 65 and over
- Without home and community-based services, the person would require nursing facility level of care
- Eligible for Medical Assistance
