Release Advance Planning (RAP)
In 2003, the Public Health Division of Human Services, in partnership with Jail Administration, implemented the concept of a Release Advance Planning (RAP) Team as an attempt to address challening inmate issues.
This team is made up of individuals from Jail Administration, Public Health Division - Family and Correctional Health Unit, Community Corrections Division, Gateway Services Division, and Community Supports Division.
The purpose of this team is to provide multi-disciplinary coordination to address anticipated inmate needs upon release. Through coordination, it is hoped that several goals will be achieved:
- Minimize challenges of the inmate leaving the correctional setting that lead to personal crisis;
- Positively impact the inmate's success upon re-introduction into the community by helping to assure basic housing, medical, and employment needs are addressed;
- Reduce the risk of re-incarceration due to unaddressed medical, housing, or employment reasons, thereby indirectly reducing the number of jail beds utilized.
This program was recognized in 2008 by the National Association of County and City Health Officials (NACCHO) as a promising practice.
