Child Care Plan
The 2018-2019 DRAFT Child Care Plan is available for public comment. Comments on the Child Care Plan will be accepted until September 29, 2017. The Child Care Plan outlines how Stearns County administers the Child Care Assistance Program (CCAP) under supervision of the Minnesota Department of Human Services (DHS). The plan is required for funds that the county receives for the CCAP. A biennial plan is required to be submitted to the state.

The Community Corrections Comprehensive Plan outlines how Stearns County Human Services administers all programs within the Community Corrections Division. The Plan is required for funds that the county receives through the Community Corrections Act. A biennial plan is required to be submitted to the state, and the progress report is provided to the state at the end of the first year of the planning period.
Limited English Proficiency Plan

The Limited English Proficiency (LEP) Plan is required document to provide a framework to ensure meaningful access to program information and equal opportunity to benefit from services for persons with hearing impairment and limited English proficiency.
A client has Limited English Proficiency (LEP) when he/she is not able to speak, read, write or understand the English language at a level that allows him/her to interact effectively with Stearns County Human Services staff. It is not always easy to identify a person with LEP. Some clients may know enough English to manage basic life skills, but may not speak, read or understand English well enough to understand some of the complex issues they may encounter within the human services systems (i.e., legal, medical, or program language).

MFIP Biennial Services Agreement Plan

Minnesota Statute Chapter 145A requires Community Health Boards to complete a comprehensive Community Health Assessment and from that, develop a Community Health Improvement Plan or CHIP. The Community Health Improvement Plan (CHIP)is a long-term, systematic effort to address public health problems in a community. The CHIP is based on the results of thecommunity health assessment (CHA) and is required to assure there is community input and engagement in the process. Once completed, this plan is required to be submitted to the Minnesota Department of Health.

The Quality Improvement Plan is a guidance document that describes processes the Public Health Division will use to manage, deploy, and assure quality in their work. More specifically, it describes the continuous and ongoing efforts to achieve measurable improvements in the efficiency, effectiveness, performance, accountability, outcomes, and other indicators of quality in services or processes which achieve equity and improve the health of all persons in Stearns County. The Quality Improvement Plan is required to be submitted to the state.
